Gummo, without a doubt. Disturbing and not in a bang-you-over-the-head-way of showing how depressing a white trash down can really be.
Grave of the Fireflies still makes me get a lump in my throat as well.

"the girl next door" based on the true story of sylivia likens. it was really disturbing to know that it actually happened to someone in real life. this movie changed the way i look at life.

Chan-Wook Park's Sympathy For Mr Vengeance is worse....
*Spoiler Alert!*
MUCH worse. Oldboy is violent, but cartoonish in many respects. "Sympathy" is...well...fucked. The main characters are played so real and honestly likable that it's truly painful to watch scenes like this.
